Introduction
The Joint Health Improvement Plan was launched in March 2004 with seven key aims:
- Identify the contribution that partners and others are already making
to improving health and reducing health inequalities.
- Use the plan to highlight good practice and identify evidence based research that can be used to inform future policy
- Assemble the main plans and strategies aimed at improving health in
South Lanarkshire together in one document
- Engage the wider public by raising awareness of health issues
- Identify priorities for future investment and funding
- Identify areas for more effective service delivery through better co-ordination and partnership
- Monitor the impact of the action identified in the plan on changes in
health outcomes for individuals and communities.
The context for health improvement has changed significantly since we produced the Joint Health Improvement Plan in 2003/04. Health improvement is now a key part of work for a number of community planning partnerships including regeneration, community safety, children and young people and the rural partnership. Community Health Partnerships (CHPs) lead the NHS approach to health improvement with an emphasis on services in the community. Community Planning has also developed and the first Key Performance Indicator (KPI) Report for the South Lanarkshire Community Planning Partnership was produced in 2006. This includes five KPIs for health.
